The issue was written by David Michelinie — whose run on Amazing Spider-Man was second in length only to Stan Lee's — with interior art by Mark Bagley, inks by Randy Emberlin, colors by Bob Sharen, and a cover by Erik Larsen, under editors Jim Salicrup and editor-in-chief Tom DeFalco. Michelinie had originally intended to kill off Eddie Brock around issue #400 and cycle the Venom symbiote through a series of new hosts, but Marvel's editorial team refused as Brock's popularity soared; Carnage was the creative solution, a host character explicitly designed without Venom's moral code. Larsen designed Kasady's human look, drawing conscious inspiration from DC's Joker, and Stan Lee reportedly pushed for the new symbiote character to be far darker and more vicious than Venom had become.
