The issue was written by James Tynion IV (main story) and Brandon Thomas (back-up), with Jorge Jiménez penciling and inking the lead story and Jason Howard handling the back-up — the same creative pairing that had been rotating through the Fear State chapters since Batman #112. Tynion had publicly stated his deliberate strategy of introducing new characters throughout his Batman tenure, noting in his newsletter that new character creation had not been a consistent sales driver in Batman books since the New 52, and Fear State was designed as the culmination of that character-building work. The issue went on sale October 5, 2021, carrying a December 2021 cover date, and was later collected in Batman: Fear State, the fifth and final trade paperback collection of Tynion's run.
