Superior Spider-Man (2023) #4 — titled 'Two Minds' — delivers the pivotal mind-war at the heart of the series' third volume: Peter Parker and Otto Octavius clash inside Peter's own consciousness for control of his body, with Peter ultimately prevailing. The issue also marks the first appearance (in illusion/holographic simulation) of the Sinister Six Hundred, a massive conceptual expansion of Doc Ock's infamous team that exists as a manifestation of Peter's haunted memories. As part of Dan Slott's 10th-anniversary return to the Superior concept, the issue explores how thoroughly Otto's tenure as Spider-Man permanently altered Peter's psychology — a meta-examination of the entire Superior Spider-Man legacy through the lens of one man's fractured mind.

The series grew out of Marvel's Sales department approaching writer Dan Slott — who created the original Superior Spider-Man saga in 2013 — to craft a 10th-anniversary storyline, which Slott has described as happening 'like clockwork.' What began as a planned arc of the ongoing Spider-Man title by Slott and artist Mark Bagley expanded into an eight-issue standalone series launching in November 2023, preceded by the one-shot Superior Spider-Man Returns. It was announced at San Diego Comic-Con 2023 that the new series would replace the Slott/Bagley Spider-Man book for eight issues, with Ellie Pyle as editor.
