John McCrea's cover depicts Etrigan decked out in a noir-styled fedora and long coat, cigarette dangling from his snarling mouth as he blazes away with two guns while shell casings fly through the air around him. It's a gloriously unexpected image — Hell's own demon gone full gangster — and it sets a darkly playful tone that makes issue #49 a genuinely fun entry in the series.
