Spawn's old nemesis, Billy Kincaid, is back as an undead spirit who possesses people and causes them to do very violent things. Billy taunts Spawn with the fact that he plans on destroying all of the people our hero holds dear. The campaign starts with the possession of a man on the subway who threatens the life of Twitch's wife, Helen, until Spawn intervenes.
