Having been shot in the head, Twitch has been languishing in a coma at the hospital for three days. Sam is praying fervently for his recovery when Spawn shows up and heals his partner. Now indebted to the creature he has been trying to capture, Sam is compelled to obey Spawn's command--find a brutal killer who has been murdering the homeless.
