Deep in the grimy underworld of New York City's alleys, a massive, spike-shouldered figure wearing a battered "I ♥ New York" shirt lunges forward with feral, glowing eyes and a wide, menacing grin — while clutching a grotesque severed zombie head in one hand. The cover art by Greg Capullo and Todd McFarlane crackles with raw, street-level menace, packing every corner with decayed skeletons, dripping sewage, and a dim hanging light that casts the whole scene in eerie shadow. "Ripples" promises the kind of dark, visceral storytelling that made Spawn one of 1995's most compelling Image titles.

  • First appearance of Helen Williams, wife of detective Twitch Williams, whose introduction gives the Sam-and-Twitch subplot a domestic dimension.
  • Cameo appearance of Tony Twist (Antonio Twistelli), the Mafia don whose name McFarlane publicly acknowledged was taken from real NHL enforcer Anthony 'Tony' Twist — an admission central to the landmark right-of-publicity lawsuit Twist filed in 1997, which ultimately settled out of court in 2007; subsequent reprints renamed the character 'Vito Gravano.'
  • The mystery figure who rescues the Violator from the sewer flood (following the battle begun in Spawn #33) is the Freak — a sewer-dwelling psychopath who goes on to play a significant antagonist role in the series; the extent of his appearance in this issue (background cameo vs. second cameo, his first being in #33) is debated among collectors.

  • Issue #34 is collected in Spawn Collection Volume 3 (issues #34–54) and was also reprinted as part of the black-and-white Spawn Compendium (issues #1–50) released in 2012 for Image Comics' 20th anniversary.
