The series was conceived as a direct continuation of Derek Landy's Doctor Strange of Asgard miniseries, picking up from the status quo established there and in Al Ewing and Pasqual Ferry's Thor run, which severed the connection between Midgard and the other Nine Realms and left Strange with no way home. Landy and artist Ivan Fiorelli (previously of Fantastic Four) were announced for the new ongoing in August 2025, with a prelude story by the same team appearing in the Doctor Strange #450 one-shot before the series launched on December 3, 2025. Cover painting duties went to fantasy illustrator Alex Horley, and colorist Dono Sánchez-Almara rounds out the core creative unit that carried through to issue #6.
