Carlos Ezquerra's cover is genuinely unsettling — a massive, horned demonic face wreathed in flames dominates the image, its fanged mouth laughing "HA HA HA HA" while the anguished faces of two human figures are absorbed into its red, hellish form. The cover text taunts Johnny Alpha directly — "You are trapped, Johnny Alpha… trapped in Hell for ever!" — promising a thrilling instalment of pulse-pounding adventure inside.
