This is not a pulp magazine. The document is a U.S. Air Force Technical Information Form FTD 164, dated October 1962, used by Project Blue Book to collect witness reports of unidentified flying objects. The form was sent to a witness in North Ridgefield, New Jersey in July 1965, requesting detailed information about their UFO sighting reported in a letter dated 3 July 1965. The questionnaire contains 35 numbered items requesting specifics: date and time of observation, location, weather conditions, object appearance (solid, transparent, or as a light), brightness, edges, motion characteristics, color, sound, estimated speed and distance, observer location, and comparative descriptions. Respondents were asked to provide sketches showing object shape, motion path, and angular size estimates, along with personal identifying information. The form states responses would be used for research purposes with witness confidentiality protected.
