This document is not a pulp magazine but rather a U.S. Air Force Technical Information Sheet (ATIC Form 129, revised September 25, 1952) designed for collecting witness reports of unidentified aerial phenomena. The form comprises detailed questionnaires requesting information about UFO sightings, including date, time, location, duration, sky conditions, object appearance, motion characteristics, and observer details. Questions systematically gather data on the object's brightness, shape changes, speed, size estimation using match-stick comparison, direction of movement, weather conditions, and witness credibility. The form instructs respondents to draw sketches showing object shape and flight path, estimate angular measurements from true north and horizon elevation, and provide personal identifying information.
