This document is a Project 10073 Record Card—a UFO incident report form used by the U.S. Air Force. The sighting occurred in Arcata, California, documented through ground-visual observation via telescope on August 9th at 10:50 PST. A civilian observer using a Rilex telescope with a Brandon Orthosconic 24mm eyepiece and Kodak Panatomic X film captured a photograph of an unidentified object. The photographer noted that while photographing with a 10-inch telescope, an object appeared in the frame; a subsequent photograph taken moments later did not capture the same object.
