This document is a U.S. Air Force questionnaire form (FTD SEP 63, Project 10073 Record) designed to collect witness reports of unidentified aerial phenomena. The form systematically gathers observational data including date, time, location, duration of sighting, sky conditions, object appearance (solid/transparent, fuzzy/sharp edges), and behavioral characteristics (acceleration, shape changes, flashing, disappearance patterns). It requests angular size estimates using a matchstick method, drawing of object shape and movement path, speed and distance estimates, observer location and vehicle details, and optical aids used. Additional sections cover multiple object sightings, prior similar sightings, other witnesses, and personal information about the observer including relevant experience. The form emphasizes that respondent names will not be used without permission and that data is for research purposes. This represents the official U.S. Air Force methodology for documenting civilian UFO reports during the early 1960s.
